Fixed an issue in the Alive Survey Definition form where the survey description and remark were not being saved when creating or editing an existing survey.

A new generic Alive format, called 'Alive_EDD_v1', has been created for submitting Alive data via Collect and Enterprise EDP that does not require separate formats for each survey type.
